These services are complementary therapies for Medical Acupuncture patients only.
Payment
Visa, Master Card, Discover, cash and local checks are accepted. All payments are due in full at the time of service. If you have insurance that covers acupuncture, a super bill will be provided so that you may submit your claims yourself. You are responsible for the billed charges in full.Cost of Care
Most conditions require an average of 6-12 treatments, although some will respond well within 4-6 visits and others may require a shorter or longer series – this depends on the severity and the chronic nature of the chief complaint.
Missed Appointments/Cancellations
A 24 hour notice is required to cancel or change your appointment. A fee of $25 will be charged for cancellations or changes with less than 24 hours notice. Email chanta@newleafacu.com or call 920-868-5108 for cancellations or changes. Your understanding is appreciated.
INCLEMENT WEATHER: If Gibraltar, Sevastopol, or Sturgeon Bay schools cancel, delay or close early, then no cancellation fee will be charged. If no schools close/delay, fees will be charged for cancellation. Please email or call EITHER WAY--to confirm or cancel--on inclement weather days.
Herbal Formula Prescriptions
Herbal Formulas are charged separately. At New Leaf, traditional Chinese herbal formulas are available in convenient tablet form for $10 per bottle. Custom "raw," or whole-herb formulas are available to cook at home yourself if you wish. The cost of whole herbs varies with the price of the herbs used. Herbs to help fight off a cold might only be required for 2-3 days, while herbs for a chronic condition such as arthritis may be desired indefinitely.
Do you accept insurance?
Insurance is not being accepted at this time. Full payment is due at the time of service. If your insurance policy covers acupuncture, a super bill can be provided so that you can submit your claim.
Will my insurance cover acupuncture?
Some insurance companies do cover acupuncture. On some policies acupuncture is included under “complementary therapies” in the category with chiropractic treatments. Contact your insurance carrier to find out if acupuncture is included in your benefits.
